Individual Contribution Standards and Enrollment Rates in Resident Medical Insurance: A Theoretical and Empirical Analysis Based on a Multiperiod Stochastic Optimization Model

This paper develops a multiperiod stochastic optimization model in which enrollment, consumption, and medical treatment decisions are made simultaneously to theoretically examine how increases in individual contribution standards affect residents' decisions to enroll in medical insurance. The study finds that, under the assumptions of a linear bequest function and that enrollment decisions do not affect treatment decisions, residents' commonly used approach to making enrollment decisions—comparing whether expected reimbursable medical expenses exceed individual contributions—is rational. Numerical simulations show that elderly residents may voluntarily withdraw from the insurance program; once the individual contribution standard exceeds RMB 450, the enrollment rate falls below 90%; and increases in individual contribution standards also raise the overall mortality rate among residents.

Abstract

本文通过建立参保、消费、疾病治疗三者同时决策的多期随机优化模型,从理论上探讨了个人缴费标准上涨对居民医保参保决策的影响。研究发现,在线性遗产函数和参保决策不影响治疗决策的假设下,居民常见的参保决策方法是理性的,即比较预期报销的医疗费用是否大于个人缴费。数值模拟结果表明:老年居民存在自发退保现象;个人缴费标准超过450元后参保率将降到90%以下;个人缴费标准上涨还会导致居民总体死亡率上升。
